RESUMO

Accurate diagnostic imaging is required for surgical planning of acute thoracolumbar intervertebral disc extrusion. However, data comparing the accuracy of conventional IV contrast-injected computed tomography (CT) and myelography CT for hemilaminectomy localization and size assessment are sparse. In this study, IV contrast-injected CT and myelography CT were performed in 48 French bulldogs presenting with acute disc herniation and compared with postsurgical CT. CT images were evaluated by three raters. IV contrast-injected CT erroneously identified the compressive lesion site significantly more often than myelography CT. The length of the compressive lesion was significantly higher using myelography CT compared with conventional CT, but this did not lead to relevant consequences on the surgical opening site length. Myelography CT should therefore be recommended when thoracolumbar disc disease is suspected and multiple compressive lesions are visualized on IV-injected CT in French bulldogs.

OBJECTIVE: To assess whether the volume of extruded materials is correlated with neurologic severity in dogs with type I thoracolumbar intervertebral disk herniation (TL-IVDH). ANIMALS: 70 client-owned small-breed dogs with type I TL-IVDH diagnosed between July 1, 2016, and June 30, 2018. PROCEDURES: For this retrospective cohort study, the medical records of 70 dogs with surgically confirmed type I TL-IVDH were reviewed. The volume and height of the intervertebral disk and the area of the maximal transverse compressed spinal cord were measured using CT myelographic images. For each dog, the volume of the disk immediately cranial to the herniated disk was an internal control. Dogs were grouped on the basis of grade of neurologic severity. RESULTS: Preoperative grades of neurologic severity were grade 2 in 7 (10%) dogs, grade 3 in 16 (23%) dogs, grade 4 in 28 (40%) dogs, and grade 5 in 19 (27%) dogs. The total volume of the affected intervertebral disks was significantly larger than the internal control. Weak positive correlation was found between the volume of the extruded materials into the vertebral canal and the grade of neurologic severity. CLINICAL RELEVANCE: Our findings indicated that the total volume of the affected intervertebral disks is larger in dogs with type I TL-IVDH, and the volume of the extruded materials into the vertebral canal is weakly correlated with the neurologic severity.

In research and academic activities, guidelines are essential and imperative especially on the use of animals. Alternative methods that do not bring academic or scientific harm should also be sought. This study aimed to develop a training model for the collection of cerebrospinal fluid (CSF) and myelography in the cervical and lumbar regions in cadavers of embalmed dogs, using an alcoholic solution and curing salts for fixation and conservation. The dogs were divided into 4 grups of 8 animal each and stored between 2ºC and 6ºC, for 30, 60, 90, or 120 days. Durotomy was performed to implant two urethral catheters (one in the cranial direction and another in the caudal direction to the spinal cord access site), in the subduraracnoid space. This space was fixed via manual infusion of saline solution with a 20-mL syringe to simulate the presence of the CSF and the positive pressure, while the puncture was made. Four cadavers of each group were randomly selected for the CSF puncture from the atlantooccipital joint and in the lumbar region between L5 and L6, respectively, and four were used for CSF puncture training, in which radiographic contrast (myelography) was injected in the same locations. This model was cost-effective, did not utilize toxic products, and can preserve cadavers for up to 120 days. In this novel anatomical model, a maximum of 15 students can be trained on CSF puncture, allowing cervical and lumbar myelography and at least 30 perforations per cadaver.


É essencial e imperioso ter critério quanto ao uso de animais em pesquisa e atividades de ensino e, consequentemente, buscar métodos alternativos que não causem prejuízo acadêmico ou científico. Para que não ocorra deterioração dos tecidos, a fixação e conservação de peças anatômicas e cadáveres devem ser realizadas. Objetivou-se, com este estudo, desenvolver um modelo anatômico para treinamento de colheita de líquido cerebroespinhal (LCE) e mielografia, nas regiões cervical e lombar. Os cães foram divididos em quatro grupos contendo oito animais cada e armazenados entre 2ºC e 6ºC, por 30, 60, 90 ou 120 dias. Foi realizada durotomia para implantação de duas sondas uretrais, no espaço subaracnóide. A infusão manual de solução fisiológica com seringa de 20 mL foi utilizada para simular a presença do LCE e a pressão positiva, enquanto era feita a punção. Quatro cadáveres de cada grupo foram selecionados para a punção de LCE na articulação atlantooccipital e na região lombar entre L5 e L6, e quatro foram utilizados para o treinamento da punção de LCE e injeção de contraste radiográfico (mielografia). A técnica anatômica empregada possibilitou o desenvolvimento de um modelo visando ao ensino e pesquisa da radiologia em cadáveres de cães quimicamente preparados, a custo baixo e sem utilização de produtos tóxicos, mantidos sob refrigeração por 120 dias. Com isso, um máximo de 15 alunos podem ser treinados em punção do LCR, permitindo mielografia cervical e lombar com 30 perfurações por cadáver.

This report describes a spinal cord epidural and synovial lipomatosis in a 3-year-old neutered male Eurasian dog. This dog presented for ambulatory paraparesis and was previously treated with immunosuppressive dosages of prednisolone for 2 years. Computed tomography (CT) myelography and magnetic resonnance imaging (MRI) images identified dorsal compression of the thoraco-lumbar spinal cord by hypertrophic epidural fat. Histological examination identified extensive well-differentiated mature adipose tissue in the subepithelial area of the tarsal synovium. Prednisolone is a reported predisposing factor in humans with lipomatosis.

Porcine models of spinal cord injury (SCI) have an irreplaceable role in the development of experimental therapies. There is little literature regarding CT myelogram (CTM) techniques in swine and morphometry in miniature swine has not been established. A CT-guided method for performing myelography as well as reference values for spinal morphometry in healthy Yucatan miniature swine is lacking. The goal of this study is to describe a CT-guided method of performing CTM in a porcine model of SCI and to establish spinal morphometric reference values in mature Yucatan pigs. Six healthy, Yucatan sows, 9 months of age, weighing between 39-57.7kg, with no history of spinal disease, spinal injury, or neurologic deficits on physical exam were used in this study. CT myelography was performed in each sow under general anesthesia. CT scout images were used to guide needle placement at the L3-L4 intervertebral site. Once correct needle placement was confirmed using a 1ml test injection, a full dose of iodinated contrast (0.3ml/kg) was injected slowly over a 2-minute time period. Morphometry was performed using area measurements of the spinal cord (SC), vertebral body (VB), dural sac (DS), and vertebral canal (VC) at the mid-body and the intervertebral disc space of each spinal segment. Of the quantitative measurements, the spinal cord surface area had the widest range of values and the greatest coefficient of variance (CV) while those parameters for the vertebral canal had a low CV. Of the morphometric ratios, the DS:VC, had the lowest CV while the spinal cord ratios to DS and VC had the highest (>30). The vertebral canal surface area and the dural space: vertebral canal ratio may serve as reference values in future studies using this animal model.

BACKGROUND: Computed tomographic myelography can be a useful tool for evaluating vertebral canal stenosis. However, an index of spinal cord compression is yet to be established. OBJECTIVES: This observational descriptive study aimed to establish an index for spinal cord compression using computed tomography (CT). METHODS: Twenty-three Thoroughbred horses (age, 155-717 days; weight, 205-523 kg) with suspected cervical vertebral malformation were subjected to computed tomographic myelography in dorsal recumbency using large-bore gantry CT to define the entire cervical vertebrae from C1 to C7. Subsequently, the height of the spinal cord was measured in the sagittal plane reformatted using curved multi-planar reformation (MPR), thereby comparing it with stenotic ratio (i.e. dividing the area of spinal cord by that of the subarachnoid space) measured in the transverse plane. The measurement was performed at the level of each of six intervertebral spaces, for a total of 138 sites. Accordingly, the appropriate cut-off value for spinal cord height was determined using the receiver-operating characteristic curve, from which the area under the curve with 95% confidence interval was estimated. RESULTS: The spinal cord compression cut-off value was 7.06 mm, with an area under curve of 0.84. A weak relationship was observed between spinal cord height and stenotic ratio (R2  = 0.08, p < 0.05). CONCLUSIONS: Following curved MPR, a cut-off value of 7.06 mm may serve as an index for spinal cord compression.

The purpose of this observational study was to determine the characteristic computed tomographic (CT) myelography findings of cervical vertebral stenotic myelopathy (CVSM) lesions in Thoroughbred horses. A total of 23 Thoroughbred horses (age range, 155-717 days on CT examination; mean, 410.9 days) were analyzed. All 23 Thoroughbred horses underwent unenhanced radiography, radiographic myelography, and CT myelography. Unenhanced radiographs were observed the presence of cervical vertebral malalignment and osseous lesions. Radiographic myelograms were observed for signs of cervical spinal cord compression; additionally, CT myelograms were used to detect cervical vertebral osseous lesions. Ventral compressions were frequently observed in the cranial cervical vertebrae (C2-C4), whereas dorsal compressions were frequently observed in the caudal cervical vertebrae (C5-C7). Furthermore, osseous lesions of the caudal articular process developed more frequently than those of the cranial articular process. CT myelography in Thoroughbred horses is a useful method for detecting CVSM changes.

OBJECTIVE: To quantify the degree of dural compression and assess the association between site and direction of compression and articular process (AP) size and degree of dural compression with CT myelography. ANIMALS: 26 client-oriented horses with ataxia. PROCEDURES: Spinal cord-to-dura and AP-to-cross-sectional area of the C6 body ratios (APBRs) were calculated for each noncompressive site and site that had > 50% compression of the subarachnoid space. Site of maximum compression had the largest spinal cord-to-dura ratio. Fisher exact test and linear regression analyses were used to assess the association between site and direction of compression and mean or maximum APBR and spinal cord-todura ratio, respectively. RESULTS: Mean ± SD spinal cord-to-dura ratio was 0.31 ± 0.044 (range, 0.20 to 0.41) for noncompressive sites and 0.44 ± 0.078 (0.29 to 0.60) for sites of maximum compression. Sites of maximum compression were intervertebral and extra-dural, most frequently at C6 through 7 (n = 10), followed by C3 through 4 (6). Thirteen horses had dorsolateral and lateral compression at the AP joints, secondary to AP (n = 7) or soft tissue proliferation (6). Site significantly affected direction of compression, and directions of compression from occiput through C4 were primarily ventral and lateral, whereas from C6 through T1 were primarily dorsal and dorsolateral. No linear relationship was identified between mean or maximum APBR and spinal cord-to-dura ratio. CONCLUSIONS AND CLINICAL RELEVANCE: CT myelography may be useful for examination of horses with suspected cervical compressive myelopathy. Degree of compression can be assessed quantitatively, and site of compression significantly affected direction of compression.

INTRODUCTION: A mixed breed dog was anesthetized for diagnostic myelography to investigate acute onset neck pain. Instead of contrast medium, 444 µg/kg medetomidine were inadvertently injected into the cerebromedullaris cisterna owing to a human error. Severe bradycardia, undetectable peripheral pulse, respiratory arrest and loss of pupillary, palpebral and corneal reflexes were observed immediately after injection. Profound hypothermia developed and esophageal temperature, measured 20 minutes after medetomidine injection, was 33 °C. Atipamezole at 1 mg/kg im was administered, followed by a second dose of 0,5 mg/kg iv 20 minutes thereafter. In the meantime, cardiorespiratory parameters and body temperature were monitored, and supportive care that included manually assisted pulmonary ventilation, active warming, and administration of 5 µg/kg/min dopamine was initiated. The dog's clinical condition improved within one hour from the beginning of supportive care, at which time ocular reflexes and swallowing returned, spontaneous ventilation was deemed as adequate and the trachea could be extubated. The dog was discharged in good clinical conditions five days later. Human error and distraction led to a potentially life-threatening complication in the dog of this report and could have possibly been prevented with the use of checklists and with a clearer definition of roles and responsibilities of the personnel involved prior to commencing the clinical procedure. Profound cardiovascular, respiratory, and thermoregulatory depression caused by intracisternal injection of medetomidine responded to parenteral administration of its antagonist and supportive care.


INTRODUCTION: Un chien croisé a été anesthésié pour une myélographie diagnostique afin d'étudier une douleur aiguë au niveau du cou. Au lieu du produit de contraste, 444 µg/kg de médétomidine ont été injectés par inadvertance dans la citerne cérébello-médullaire en raison d'une erreur humaine. Une bradycardie sévère, un pouls périphérique indétectable, un arrêt respiratoire et une perte des réflexes pupillaire, palpébral et cornéen ont été observés immédiatement après l'injection. Une hypothermie profonde s'est développée et la température oesophagienne, mesurée 20 minutes après l'injection de médétomidine, était de 33 °C. De l'atipamézole à 1 mg/kg im a été administré, suivi d'une seconde dose de 0,5 mg / kg iv 20 minutes après. Dans l'intervalle, les paramètres cardiorespiratoires et la température corporelle ont été surveillés et des soins de soutien comprenant une ventilation assistée manuellement, un réchauffement actif et l'administration de 5 µg/kg/min de dopamine ont été initiés. L'état clinique du chien s'est amélioré dans l'heure qui a suivi le début des soins, moment où les réflexes oculaires et la déglutition sont réapparus, la ventilation spontanée a été jugée adéquate et où on a pu procéder à l'extubation. Le chien est sorti dans de bonnes conditions cliniques cinq jours plus tard. Une erreur humaine et de la distraction ont conduit à une complication potentiellement mortelle chez le chien décrit dans ce rapport et auraient pu être évitées grâce à l'utilisation de listes de contrôle et avec une définition plus claire des rôles et des responsabilités du personnel impliqué avant le début de la procédure clinique. Une profonde dépression cardiovasculaire, respiratoire et de la thermorégulation causée par l'injection intracisternale de médétomidine a répondu à l'administration parentérale de son antagoniste et à des soins de soutien.

An 8-year-old Warmblood gelding presented with a history of progressive ataxia for 6 weeks. Intra- and intervertebral ratios measured from lateral radiographs of the cervical spine were within normal limits. Computed tomographic myelography of the cervical spine revealed focal compression of the dorsal and the ventral contrast column as well as a ventral displacement of the spinal cord within the spinal canal due to a bulging of soft tissue attenuating material in the dorsal half of the intervertebral junction of C6 and C7. Post-mortem histopathological examination confirmed chondroid metaplasia of the ligamentum flavum at C6-C7.

Background: The techniques described for the identification of the lumbosacral (LS) epidural space in dogs do not guarantee the needle position or an accidental subarachnoid puncture, especially in small size dogs. Aim: To determine the relationship between body weight and the location of the dural sac (DS) using myelography in dogs, and to determine the possibility of subarachnoid puncture during LS epidural based on the position of the DS. Methods: Four masked observers evaluated 70 myelographic studies of dogs, annotating the vertebrae where the DS ended, if it was localized before or after the LS space, and if accidental subarachnoid puncture during LS epidural injection was possible (yes/no). Body weight (kg) was categorized into: less than 10 kg, between 10 and 20 kg, and more than 20 kg and was also converted to body surface area (BSA) as a continuous variable. Results: The DS ended at the LS space or caudally in 50% of dogs. There was a statistically significant difference between the position of the DS and the dog's BSA (p = 0.001). The DS ended caudal to the LS space in 72.7% of dogs weighing <10 kg, in 25% of dogs between 10 and 20 kg and in 15% of dogs in the >20 kg category. The observers considered a possible subarachnoid puncture during LS epidural in 69.7% of patients <10 kg, 16.6% on those between 10 and 20 kg, and in 11.7% of the dogs >20 kg. Conclusion: The DS ended caudal to the LS space in almost 3/4 dogs in the <10 kg category, so accidental subarachnoid puncture during LS epidural is highly possible in this weight range.

BACKGROUND: Gastroesophageal reflux (GER) has been reported to be a common finding in dogs under general anesthesia. OBJECTIVES: The aim of this retrospective study was to assess the esophageal and gastric contents in a population of dogs undergoing computed tomographic myelography (myelo-CT) examination and to evaluate the factors influencing the presence of esophageal fluid (gastric content, duration of anesthesia, body position, and intrinsic factors). METHODS: Esophageal and gastric contents of 83 non-brachycephalic dogs were retrospectively assessed based on plain and myelo-CT scans. Age, weight, breed, sex, and the time between the 2 computed tomography [CT] scans were included. RESULTS: Esophageal fluid was present in 19% (16/83) of the animals, and 14% (12/83) and 46% (37/83), respectively, had fluid or food material in their stomachs. The frequency of observing esophageal fluid on myelo-CT scans was significantly increased compared with plain CT scans (p = 0.006). The presence of gastric fluid was significantly associated with an increased frequency of observing esophageal fluid compared to other gastric contents (p = 0.049; odds ratio, 3.1). The presence of esophageal fluid was not correlated with alimentary gastric contents (p = 0.17). Increased body weight and duration of anesthesia were significantly associated with an increased frequency of observing esophageal fluid (p = 0.022, p = 0.021). CONCLUSIONS: Unlike alimentary gastric contents, fluid gastric contents were correlated with the presence of esophageal fluid upon myelo-CT. The observation of fluid in the esophagus may be consistent with GER. This study provides data additional to pH monitoring studies of GER and may support previous studies recommending shorter pre-anesthetic fasting periods in dogs.

O objetivo deste trabalho foi verificar em quais projeções foi possível identificar compressão da medula espinhal em cães com doença do disco intervertebral (DDIV) cervical e propor um sequenciamento das projeções a ser realizado no exame mielográfico dessa região. Foram avaliadas quatro projeções mielográficas (lateral, ventrodorsal e oblíquas esquerda e direita) de 41 pacientes diagnosticados com DDIV cervical. Em 40 pacientes (97,5%), foi possível identificar compressão da medula espinhal na projeção lateral; em 22 (53,6%), nas oblíquas; e em 11 (26,8%), na ventrodorsal (P<0,05). Havia lateralização da compressão em 22 (53,6%) pacientes; 100% delas (n=22) foram detectadas pelas projeções oblíquas e 50% (n=11) pela ventrodorsal. Em 10 (24,4%) cães, foi observado mais que um local de compressão, tendo as projeções ventrodorsal e oblíquas auxiliado na definição do local de compressão em 50% e 70%, respectivamente. Pode-se concluir que todas as projeções mielográficas estudadas permitem identificar compressão na medula espinhal em cães com DDIV cervical, sendo a incidência lateral a que mais a revelou, seguida das oblíquas e da ventrodorsal, estabelecendo-se, assim, uma proposta de sequenciamento das projeções mielográficas a serem realizadas para essa região.(AU)


The aim of this study was to verify in which of the myelographic views it was possible to identify spinal cord compression in dogs with cervical intervertebral disc disease (IVDD), and to establish a sequence in which myelographic views should be obtained for this region. Four myelographic views (lateral, ventrodorsal, left oblique and right oblique) of 41 patients diagnosed with cervical IVDD were evaluated. In 40 patients (97.5%) it was possible to identify spinal cord compression by lateral view, 22 (53.6%) by the oblique view, and 11 (26.8%) by the ventrodorsal view (P< 0.05). There were lateralized compressions in 22 (53.6%) patients, detected by all oblique views (100%) and by 11 (50%) of the ventrodorsal views. In 10 (24.4%) dogs, more than one compression site was observed, where the ventrodorsal view helped to decide the site in 50% of the cases and oblique in 70%. It can be concluded that all the tested myelographic views allow the identification of spinal cord compressions in dogs with cervical IVDD, the lateral view being the most relevant, followed by the oblique and ventrodorsal view, therefore establishing a sequence of myelographic views should be obtained for this region.(AU)

BACKGROUND: Three-dimensional computed tomographic (CT) evaluation of the cervical vertebral column enables more accurate identification of osseous and soft tissue lesions than traditional latero-lateral radiography. However, examination of the complete cervical vertebral column has been limited by horse size, preventing evaluation of the caudal cervical vertebrae. OBJECTIVES: To describe a technique to enable CT myelography of the complete cervical spine and describe the findings in 51 horses. ANIMALS: Records of 51 horses presented for evaluation of cervical vertebral lesions. METHODS: A retrospective review of clinical records from all horses presented for CT myelography to further investigate possible cervical vertebral lesions was performed. A description of a novel approach to CT myelography in horses and retrospective review of the findings in clinical cases has been included. RESULTS: Degenerative joint disease was identified at 1 or more dorsal articular process joint in 50/51 horses, of which 44/51 had a site of grade 2 or greater. Spinal cord compression was observed on CT myelography in 31/51 horses, whereas attenuation of the dorsal contrast column was identified radiographically in 11/50 horses. Thirty-three horses showed narrowing or obliteration of the intervertebral foramina at 1 or more site and osteochondral fragments were seen in 11/51 horses. CONCLUSIONS AND CLINICAL IMPORTANCE: Computed tomography myelography is relatively safe and an easily performed technique with the correct equipment, enabling evaluation of the cervical vertebral structures of horses in all planes and volumetrically. It is possible that lesion extent might be underestimated with this diagnostic modality, hence interpretation should be complimented with flexed and extended views radiographically.

A 9-year-old neutered male Rhodesian ridgeback cross dog was evaluated for progressive non-ambulatory paraparesis, fever, and leukocytosis. The dog was diagnosed with spinal epidural empyema (SEE) and infectious endocarditis (IE) of the mitral valve based on the findings of contrast-enhanced computed tomography (CT), CT myelography, echocardiography, and bacterial culture. The report herein describes the clinical presentation, CT findings, clinical and surgical management of this case, together with the electrocardiography, and echocardiography findings. To the authors' knowledge, this is the first reported case of spinal epidural empyema likely to be caused by infectious endocarditis of the mitral valve in a dog.


Empyème épidural spinal concomitant à une endocardite chez un chien. Un chien mâle castré croisé Rhodesian Ridgeback âgé de 9 ans a été évalué pour une paraparésie progressive non-ambulatoire, de la fièvre et une leucocytose. Un diagnostic d'empyème épidural spinal (SEE) et d'endocardite infectieuse (IE) de la valvule mitrale a été émis basé sur les trouvailles de la tomodensitométrie (CT), d'une myélographie CT, de l'échocardiographie, et de la culture bactérienne. Le présent rapport décrit la présentation clinique, les trouvailles de CT, la gestion clinique et chirurgicale de ce cas, de même que les trouvailles par électrocardiographie et échocardiographie. À la connaissance des auteurs, ceci représente le premier cas rapporté d'empyème épidural spinal à être causé par une endocardite infectieuse de la valvule mitrale chez un chien.(Traduit par Dr Serge Messier).

A leucemia linfoblástica aguda (LLA) é uma enfermidade de origem linfóide e consiste na proliferação de células neoplásicas na medula óssea. O objetivo desse trabalho é relatar o caso de um cão macho, sem raça definida, de apenas um ano de idade, atendido no Hospital Universitário de Medicina Veterinária Prof. Firmino Mársico Filho (HUVET) da Universidade Federal Fluminense (UFF) com queixa principal de inapetência e diarreia há três dias e que foi diagnosticado com essa neoplasia por meio da sintomatologia clínica, resultados do hemograma e do mielograma. O paciente apresentava valores exacerbados de linfócitos (553.094 células/µL), além de anemia, trombocitopenia, hipoalbuminemia e elevação da atividade das enzimas fosfatase alcalina e ALT. Foram observadas manchas de Gümprecht, linfócitos atípicos apresentando anisocitose, anisocariose, intensa basofilia citoplasmática e monócitos ativados. O mielograma apresentou também um aumento de linfócitos e contagem de linfoblastos superior a 30% na medula, confirmando o diagnóstico de leucemia linfoblástica aguda. Ademais, posteriormente, foi realizado exame de Reação em Cadeia de Polimerase (PCR) para rearranjos de receptores de antígenos e foi detectado clonalidade para linfócitos T. O animal foi submetido à quimioterapia (protocolo com ciclofosfamida, vincristina e prednisona), mas não resistiu à gravidade do quadro, vindo a óbito após a primeira sessão, pouco tempo após o diagnóstico.


Acute lymphoblastic leukemia (LLA) is a disease with a lymphoid origin and consists of the proliferation of neoplastic cells in the bone marrow. The aim of this study was to report the case of only one year old mixed breed male dog, attended at the University Hospital of Veterinary Medicine Prof. Firmino Mársico Filho (HUVET) from Universidade Federal Fluminense (UFF), with major complaint of inappetence and diarrhea three days ago and which was diagnosed with this neoplasm through clinical symptoms, complete blood count and myelogram results. The patient had increased values of lymphocytes (553,094 cells/μL), in addition to anemia, thrombocytopenia, hypoalbuminemia and elevated alkaline phosphatase and ALT activities. Gümprecht shadows, atypical lymphocytes presenting anisocytosis, anisocariosis, and severe cytoplasmic basophilia and activated monocytes were observed. Myelogram also showed an increase in lymphocytes and a lymphoblastic count greater than 30% in the marrow, confirming the diagnosis of LLA. In addition, polymerase chain reaction (PCR) for antigen receptor rearrangements was performed and clonality for T lymphocytes was detected. The animal underwent chemotherapy (protocol with cyclophosphamide, vincristine and prednisone), but did not withstand the severity of the disease, coming to death after the first session, shortly after diagnosis.

An 11-year-old male Miniature Dachshund was referred for acute neurological deficits in the pelvic limbs. T2-weighted magnetic resonance imaging revealed that the spinal cord at the L1-2 intervertebral disc space was heterogeneously hyperintense in the sagittal plane and was mildly compressed from the ventral side by a small hypointense mass in the transverse plane. However, the lesion showed mass enhancement and severe spinal cord compression on post-contrast T1-weighted imaging. On three-dimensional myelography, a "golf tee sign" was observed around the mass. Therefore, we diagnosed an intradural extramedullary lesion. The mass was surgically removed and histologically diagnosed as a hemangiosarcoma. The "golf tee sign" observed on magnetic resonance myelography may be useful for distinguishing intradural extramedullary masses from intramedullary masses.

Due to the scarcity of myelogenous studies in cattle, the present study aimed to evaluate the efficacy and distribution of iopamidol and iohexol contrast agents in calves, in order to determine guidelines for obtaining diagnostic radiographs of spinal cord disorders in these animals. Ten healthy Holstein calves, seven days to two months of age, were divided into two groups, according to the contrast medium applied. Myelographic studies of the spine were performed with the calves in lateral recumbency, with radiographs repeated 20 times during a two-hour period. On the radiographs, the contrast medium was analyzed for opacity, detail of the image, distension of the medullary canal, and progression of the contrast line. After seven days, the myelographic studies were repeated, with the contrast media exchanged between the groups. There were no significant differences in the quality of the images and speed of the spinal column filling between the two contrast media. Furthermore, the best quality radiographic images were obtained six to eight minutes after injection of the contrast in the cervical spinal segment, 80 minutes in the thoracic, and 20 minutes in the lumbar, sacral, and cauda equina segments.(AU)


Devido à escassez de estudos mielográficos em bovinos e relatos de complicações no procedimento, o presente estudo teve por objetivo avaliar a eficácia e a distribuição dos meios de contraste iopamidol e ioexol em bezerros, a fim de nortear a melhor conduta para o diagnóstico de afecções vertebrais e medulares nesses animais. Foram utilizados 10 bezerros Holandeses, hígidos, com idade entre sete dias e dois meses, distribuídos em dois grupos, conforme o meio de contraste aplicado. O estudo mielográfico da coluna vertebral foi realizado na posição laterolateral, repetido em 20 momentos, durante o período de duas horas. Nas radiografias, analisou-se o meio de contraste quanto à opacidade, detalhes da imagem, distensão do canal medular e progressão da linha de contraste. Após sete dias, foi realizado o segundo período experimental, que compreendeu a troca do meio de contraste dentro de cada grupo. Não houve diferenças significativas em relação à qualidade da imagem e à velocidade do preenchimento da coluna medular entre os dois meios de contraste. A partir da administração dos meios de contraste, a obtenção de imagens radiográficas de melhor qualidade deu-se após seis a oito minutos no segmento medular cervical, 80 minutos no torácico e 20 minutos nos segmentos lombar, sacral e cauda equina.(AU)

Medical records (2004-2016) of five dogs with a thoracolumbar spinal arachnoid diverticula (SAD) that was diagnosed with stress myelography in four dogs and magnetic resonance imaging in three, and who had hemilaminectomy, diverticular marsupialization, and vertebral stabilization, were reviewed. Data on previous treatment, pre- and postoperative neurologic status, diagnostic findings, surgical techniques, and outcomes was retrieved. Follow-up clinical and radiographic evaluations were performed immediately; ∼1, 2, and 6 mo postoperatively; and at annual follow-up examinations. The stress myelography demonstrated spinal cord dynamic compression in three of four dogs and change in size or shape of the SAD in all four. Two dogs who had SAD recurrences 4 and 13 mo after previous surgical dural fenestration, and one dog with no previous SAD treatment demonstrated long-term neurological improvement after vertebral stabilization (49, 77, and 126 mo). In two other dogs, recurrence of clinical signs was observed at the follow-up (8 and 12 mo). This case series suggested that repetitive spinal cord injury from the dynamic lesion appears to be one potential cause of thoracolumbar SADs. In cases with dynamic lesions confirmed by stress myelography, vertebral stabilization with conventional techniques is indicated to prevent SAD recurrence.
